Terms and conditions for ASH/Active & Fit Insurance Program

These Terms and Conditions ("Terms") govern your membership with FIT4MOM and your access to an American Specialty Health (ASH) Membership ("Membership"). By enrolling in the Membership, you agree to abide by these Terms, as well as the terms and conditions of your ASH membership, and any additional terms and policies provided by FIT4MOM.

1. Eligibility: To be eligible for an ASH Membership with FIT4MOM, you must meet the following criteria:

a. Maintain an active membership with one of the following health and fitness programs: Silver & Fit, Active & Fit Now, Active & Fit Direct, or Active & Fit Enterprise ("Eligible Health and Fitness Programs"). You must ensure that your membership with the Eligible Health and Fitness Program remains active for the duration of your ASH Membership with FIT4MOM.

b. Comply with any specific eligibility requirements or guidelines set forth by FIT4MOM and ASH.

2. Duration of Membership

Your ASH Membership with FIT4MOM is valid as long as you maintain an active membership with one of the Eligible Health and Fitness Programs mentioned in Section 1(a). If your membership with any of these programs is terminated or expires, your ASH Membership with FIT4MOM will also be terminated automatically.

3. Benefits of ASH Membership

As a member of FIT4MOM with an ASH Membership, you are entitled to unlimited access to FIT4MOM classes, including Body Boost, Body Ignite, Fit4Baby, Strides 360, Stroller Barre, and Stroller Strides at participating FIT4MOM locations.

The specific benefits and services may vary by location and are subject to change at FIT4MOM's discretion.

4. Membership Termination

FIT4MOM reserves the right to terminate your ASH Membership at any time without prior notice, in its sole discretion, if you violate any of these Terms or any terms and conditions of the Eligible Health and Fitness Programs. FIT4MOM may also modify, suspend, or terminate the ASH Membership program itself at any time.

By enrolling in the FIT4MOM ASH Membership, you acknowledge that you have read, understood, and agree to these Terms and Conditions. Failure to comply with these Terms may result in the termination of your Membership.
